According to this VNUnet article Great North Eastern Railway "inadvertently printed system passwords in a magazine available to thousands of passengers".Because a picture of their control room showed a white board with their mainframe passwords written on it.
While there has been some debate online about writing down passwords, I don't think anyone has suggested writing them up - on the wall.
