CSO Online has published a tutorial we wrote on Adding Two-factor Authentication through Windows Server 2009 Network Policy Server.
We chose to demonstrate how to run your SSH logins through Active Directory to show how companies can configure authentication across operating systems using standard protocols. The key protocol here is radius. If you have standardized on Radius, then configuring any remote access tool from a VPN to Terminal Server should be quite simple.
We maintain a set of tutorials on how to add two-factor authentication to a variety of remote access applications here.
